TESTENGEER, INC. BENEFIT PLANS

 

Medical Benefit Plans

Testengeer sponsors two self-funded medical plans for regular, full-time employees following a ninety (90) day waiting period.  Both plans are major medical plans with an initial deductible, a period of employee and company co-pays reaching a stop-loss threshold where a third-party insurer pays the remaining eligible expenses for the current calendar year.  Plan A is a high deductible, low premium plan with a Health Savings Plan option.  For 2006 the Company will match an employees contribution to their Health Savings Account up to $600.00 annually.  Plan B is a lower deductible, higher premium plan that includes a pre-deductible co-pay for a physician’s visit and a prescription card. Dental Benefit Plan

The ‘PAID DENTAL-THE INCENTIVE PLAN’ is sponsored by Testengeer and underwritten by Kansas City Life. All regular, full-time employees are eligible following a ninety (90) day waiting period. The plan is a dental assistance plan covering the four basic categories of dental care – Preventative, Corrective, Restorative & Orthodontia.. Initial plan coverage begins at a basic level reaching a maximum coverage the third year. Maximum benefit payment is $1,000.00 per year.

Flexible Spending Plan

The Flexible Spending 125 Plan provide an opportunity to open medical care and dependent care accounts to assist in paying for those expenses not covered by other plans or programs.  Regular, full-time employees are eligible following a ninety (90) day waiting period.  Annual pre-tax contributions of  $2016.00 for unreimbursed medical expenses and $5,000.00 for dependent care can be authorized.  Any contributions not used within the calendar year are forfeited.

Payroll Savings Plan

Testengeer provides a 401(k) Plan & Trust to encourage employees to invest in their personal retirement plans.  All regular employees may enroll in the plan on their first day of employment. The Company provide a 50% match on the employees’ first 6% of pre-tax contributions.  The Hartford Life Insurance Companies is the administrator of the Testengeer 401(K) Plan & Trust.

Profit Sharing Plan

Testengeer has an established profit sharing plan for all employees paying up to 5% annual earnings.  Any profit sharing payments are made into the 401(k) Savings Plan.
Profit sharing payments are reviewed, authorized and paid at the discretion of the Board of Directors.  To be eligible for profit sharing, an employee must be actively employed on December 31st and worked more 1,000 hours during the year.

Life Insurance with AD&D and Long-Term Disability

Testengeer provide a term life insurance policy in the amount of $50,000 with a double indemnity accidental death rider and a long term disability policy.  Regular full-time employees are eligible for both plans on the first of the month following a sixty-day waiting period.  LTD payments replace earnings at 60% of base earning and the initial payment is made following 60 days of continued disability.

Paid Time Off

Testengeer has no officially established holidays or days of vacation.  All regular, full-time employees earn 6.154 hours of paid time off each pay period.  This rate accumulates to the equivalent of 160 hours of paid time off annually.  As paid time off is accrued employees may take paid time away from work.

Educational Assistance

To encourage the personal development of our employees, Testengeer provides 100% reimbursement of tuition, books and laboratory fees to regular full-time employees who successfully complete pre-approved courses.
